Medical and Emotional Distinctions
Miscarriage is the natural loss of a pregnancy before 20 weeks of gestation, occurring in roughly 10–20% of known pregnancies, usually due to biological factors.1 Confusion sometimes arises over medical terminology, as a miscarriage can be called a “spontaneous abortion.” However, it’s not the same as an elective abortion.
An elective abortion, on the other hand, is the intentional termination of pregnancy, either through drugs or a surgical procedure.
Key distinctions include:
- Cause: Miscarriage occurs naturally; abortion is a medically or surgically intentional decision.
- Interventions: Some miscarriages require medical procedures to prevent infection and any complications; abortions use FDA-approved drugs (mifepristone and misoprostol) or procedures to intentionally end and remove or expel the pregnancy.2
- Timing: Most spontaneous pregnancy losses happen early, with about 80% occurring in the first trimester and rates decreasing as gestational age increases;3 medical abortions are typically performed up to 10 weeks, with surgical abortions occurring later as laws allow.
- Medical supervision: Miscarriages may need intervention if tissue is not fully expelled to prevent infection and complications. An abortion is a serious medical procedure that should have medical oversight from beginning to end.
- Emotional context: Both miscarriage and abortion can involve complex emotions. Miscarriage often brings grief over a lost pregnancy, while abortion can prompt feelings ranging from relief to regret, sadness, or guilt.4
